A5 size, individually numbered, and with duplicate copies. 50 test records (and duplicates) per book. White top copy is for collector/ screener/employer and the second copy (yellow) is for the test subject/employee as evidence that may be produced as required/requested.

Suggested use: hard copy evidence of routine surveillance, trade or contractor evidence entry to site, entry to rest home facilities or other high risk areas.

Use for this form/record/book does not apply to its use as evidence for an unvaccinated person to cross borders or board domestic flights as the only acceptable test process/record for this is a free rapid antigen test performed by a Pharmacy and reported by a text sent through the Sysmex Eclare software system from that Pharmacy or a PCR test record.
